Dear CCDP-D Certificate Holder:
Your Certified Co-occurring Disorder
Professional-Diplomate (CCDP-D) certificate expires on October 3,
2010. Attached to this message are four important re-certification
documents:
Letter explaining the re-certification process;
Requirements for re-certification document;
Re-certification Application Form;
Education Summary Form.
I
n addition, a LASACT membership
application form is attached for those who would like to either
join or renew their membership to get the discounted member rate
to re-certify. During the last quarter of the year, we pro-rate
dues so that we can offer membership at a special rate of
$112.50 to cover the remainder of 2010 through 12/31/2011. By
joining at this rate you can re-certify in both 2010 and 2011 at
the member rate of $60, a saving of $30 per year. Additionally,
you will get discounted membership rates on the 2011 Conference
and any LASACT educational offerings and other certificates.
Read this letter and all
attached documents thoroughly and carefully so you will know
exactly what you need to do to re-certify.
Pay particular attention to the information
about the continuing education hours that you must submit.
All hours must be in integrated services,
dual diagnosis, and/or co-occurring disorders. Nothing else will
be accepted. Although all CCDP-D’s were told of this
requirement when they received their original certification, our
experience has been that many people have ignored this requirement
and have found themselves in a dilemma at re-certification time.
If you don’t have enough hours now, you have time to acquire them
before your certificate expires on October 3, 2010. On the LASACT
web site,

- on the home page and again under the tab to "Educational
Opportunities"- you can find a link to the Distance Learning
Center for Addiction Studies (DLCAS), which offers online courses
in co-occurring disorders and integrated services that will meet
this requirement.
If your application is not in the LASACT office
or postmarked by October 3, 2010, add a $50 late fee to the
regular re-certification fee. After
October 3, you have a 90 day grace period - until January 3, 2011
- to get the required materials in with the late fee. However,
education hours may not be acquired during this period.
IF YOUR JOB AND/OR SALARY REQUIRE
RECERTIFICATION EXACTLY ON 10/3/2010,
GET YOUR APPLICATION TO US BY SEPTEMBER 5, 2010, WITH A NOTE
REQUESTING THAT WE EXPEDITE IT FOR YOUR WORK REQUIREMENT.
OTHERWISE, PROCESSING OF
RE-CERTIFICATIONS REQUIRES AT LEAST 30 WORKING DAYS. DO NOT
CONTACT US ON 10/3/10 ASKING FOR YOUR NEW CARD IF YOU HAVEN’T
NOTIFIED US IN ADVANCE OF THE NEED FOR IT BY THAT EXACT DATE.
All of the documents attached to this message
will be posted on the LASACT web page in the next week.
